What is Piriformis/Deep Gluteal Syndrome?

Deep inside the gluteal region, there are a set of muscles that help to rotate your leg. However, because they run right over the sciatic nerve, they tend to compress the nerve and lead to symptoms commonly known as “sciatica”.

What are the main causes of Piriformis/Deep Gluteal Syndrome?

There are various reasons why piriformis syndrome can occur, some include: 

  • Tightness in the piriformis/external rotators due to compensation (weak glutes) causing them to compress the sciatic nerve 

  • Trauma directly to the gluteal region 

  • Long term repetitive strain or overuse of the muscles causing them to compress the sciatic nerve 

  • Anatomical variation that causes the sciatic nerve to pierce through the piriformis muscle and be more susceptible to compression

What are the symptoms of Piriformis/Deep Gluteal Syndrome?

  • Numbness or tingling in the gluteal region  

  • Pain that is worse with prolonged sitting/standing, squatting, or climbing stairs 

  • Sharp/shooting pain that radiates down the leg 

  • Tenderness in the gluteal region 

  • Low back pain
